Your unit was informed by some local people about hidden weapons in the city of Erbil and the soldiers from your unit were tasked with recovering them. During the action something went wrong. Write a memo to your CO about it, in which you:

Subject: Recovery operation – security incident

  1. This memorandum provides information regarding a recovery operation conducted in the city of Erbil on 10 January following reports from local residents about hidden weapons in a rural area within our sector of responsibility. Based on the intelligence received, a patrol was dispatched to verify the information and secure the suspected cache. Upon arrival at the indicated location, the soldiers initiated a standard search procedure and identified a concealed storage site containing small arms and ammunition.
  2. During the securing process, an explosive device hidden in close proximity to the cache was unintentionally triggered. Although no fatalities occurred, two soldiers sustained minor injuries. Preliminary assessment suggests that the area had not been thoroughly checked for secondary threats prior to approaching the weapons. As a result of the incident, the operation was temporarily halted and an Explosive Ordnance Disposal (EOD) team was called to the scene to neutralise additional hazards.
  3. In order to improve the effectiveness and safety of similar operations in the future, it is recommended that all recovery missions be preceded by mandatory EOD support and a detailed risk assessment. Furthermore, additional training in counter-IED awareness should be conducted for all patrol members. Strengthening cooperation with local informants and improving intelligence verification procedures would also contribute to reducing operational risk during such actions.
